What does VCFRONT-A195 mean?

Tesla alert code in the Vehicle Controller Front (VCFRONT) module. We are not allowed the vehicle to enter the drive state because the 12V battery voltage is too high. When the 12V battery voltage is measured above 16V for 3 consecutive seconds. When the 12V battery voltage is measured below 16V. Delay in entering the drive state, 12V regulation issues.
